This case is one that Dominic presented at the BDA. It shows the synergy of Bioclear composites, implants and the Dahl concept – which can work well for localised anterior tooth wear.
We are focussing on correcting poor aesthetics through clinical dentistry. The anterior composites are falling apart, some teeth are in the wrong place and there is bite collapse with anterior tooth wear with no room to restore. Dominic used restorative lead implant planning- firstly a localised CBCT scan was completed – then he shows planning abutment and crown on the image to determine angulation/angle correction to and the type of screw/cement restoration needed.
Scanning digital impressions is now the modern way (it is very rare to do conventional impressions) because it gives you instant information on the case without waiting for the lab.
After the scan, abutments come back from the lab and are screwed in with location jigs.
These abutments can now be converted into teeth using Bioclear 360º wrap around matrix, heated monolithic injection and cutback technique. Using Bioclear creates an excellent long lasting aesthetic result.
The overbuild of the Dahl effect opened up the OVD posteriorly. The teeth then super-erupt into a new occlusion. The Bioclear composites create a superb aesthetic result.
These techniques are taught in the DHTI courses.
